Getting Paid and Enrolling in Benefits (if applicable)

There are several forms/documents that you must complete upon employment at the University in order to get paid. There is no need to wait for your first day on the job to process these forms. Feel free to get a head start today by clicking on the links for the individual forms and documents under the related headings listed below. Please do not delay in completing this paperwork. Your pay and benefits cannot be processed without these important documents.
There are two sections with forms:  Section I - Payroll and Section II Benefits.

SECTION I - PAYROLL FORMS

Forms to be completed:
I-9 (477kB)  (Required form - Verifies that individuals are eligible to work in the United States)

  • Federal law requires all employees must provide verification that they are eligible in the United States. This is done by bringing the I-9 form with you on your first day and completing in person with an Human Resources representative within three days of hire. You will be asked to provide two or more forms of original identification. Either one document from List A, or one each from List B and List C as outlined on page 9 of the I-9 Form. The most common forms of documentation are:

    • U.S. Passport (unexpired or expired)
    • Permanent Resident Card or Alien Registration Receipt Card (Form I-551)
    • An unexpired foreign passport with a temporary I-551 stamp
    • Driver's License
    • Social Security Card
    • Birth Certificate

 

  • W-4 Federal Tax Form 2014 (122kB)(Federal Income Tax Withholding Certificate)
  • W-4 Michigan State Tax Form (37kB) (Michigan Withholding Exemption Certificate)
  • SW-4 (35kB) (City of Saginaw Income Tax Withholding Certificate) This form is only completed if you are a resident of the City of Saginaw.

  • FERPA Faculty Form (58kB)
    SVSU faculty is required to comply with the Family Educational Rights and Privacy Act of 1974 (FERPA). The university must provide each faculty member with a copy of this policy and obtain written acknowledgement of receipt by signing the Acceptance if Access and Responsibility Form. Student information through the Cardinal Direct and Colleague systems will not be accessible until this document is signed.

New Employee Orientation

The New Employee Orientation, conducted by Human Resources, provides a comprehensive introduction to employment at SVSU, including employee benefits.

New faculty, administrative professional, support staff, police and facilities employees should contact the Manager of Benefits at (989) 964-7100 or email jkoboyle@svsu.edu to schedule an appointment to review the completed payroll/benefit enrollment forms.

New employees should bring the following information to the orientation:

  • Information about family members to be included in insurance coverage (spouse/dependents), including name, Social Security number(s), and date(s) of birth;
  • Xerox copy of marriage license and/or birth certificates for covered dependents;
  • Beneficiary information including name, address, Social Security number(s), and date(s) of birth;
  • Original Identification(s) for completing the I-9 Form (for a list of approved documents, click here)

Benefits will be effective the first day of the month following your effective date of employment.
